Q. Do I have to do the monthly automatic withdrawal?
A. Yes.
Q. Why do I have to do the automatic withdrawal?
A. Automatic withdrawals are done as a way to keep the cost of your membership at its lowest possible rate.
Q. When will my account be debited?
A. Your account will be debited on the 15th of each month. If the 15th is on a weekend or banking holiday it will be withdrawn on the last business day prior to the 15th.
Q. When will my first bank withdrawal occur?
A. Your first bank withdrawal will be determined by your effective date. Your effective date is the next business day following the date your application is received, and not the date your application is submitted.If your effective date falls on or before the 10th of the month, your first withdrawal will be the 15th of the following month (ex. Effective Date = Jan. 9th First W/D = Feb. 15th).

If your effective date falls on the 11th of the month or after, your first withdrawal will be the 15th of the month following next (ex. Effective Date = Jan. 11th First W/D = Mar. 15th).

Q. What do I do if my banking information changes?
A. We will need your change of information by the first day of the month. Please contact an Affordable Care Solutions Representative at (417) 890-5550.
Q. When is the first payment due?
A. At the time of sign up we will collect your first two months of membership.
Q. How long is the contract term?
A. You can cancel at any time after your first two months.
Q. Can I pay for someone else to be a member?
A. Yes, you may pay membership dues for a member outside of your home, if they are under their own membership plan.
Q. How do I cancel my membership?
A. Membership cancellations must be received by the 8th of the month in order to stop your automatic withdrawal for the 15th.
Two ways to cancel your membership;

  • 1. A certified letter must be sent to 4049 S. Campbell, Springfield, MO 65807
    • a.Must list the cancelling member’s name, DOB and reasoning for cancelling.
  • 2. You can go online to cancel your membership @ www.fmwic.com
Note: Letters and online requests received after the 8th of the month will have their auto withdrawals stopped on the 15th of the following month
Q. If I cancel my membership can I rejoin later?
A. If you cancel and rejoin within the first twelve months after termination there will be a $110.00 reactivation fee plus your first two months membership dues per individual.
