Driver’s License
Health Insurance Card
Form of Payment: Cash or Check or Credit/Debit Card
Names of any Prescriptions you Take
Know which Pharmacy you want to use

Step 3

We’ll take the time to answer any questions you may have regarding your visit or health care. After your visit, post visit instructions and follow-up care information (or specialist referrals) will be provided to you.
